The Support Service will be delivered to disabled adults with the aim of increasing participation, independence, resilience and life skills through peer support. Potential Providers will have the option to offer services to all described under the Contract but do not have to be able to offer to all groups / ages and can specialise in any. This Service Specification describes the requirements for support that is delivered to people individually. Community Networks are designed around several individuals in independent properties (of any tenure type), ideally within walking distance of each other, in a sustainable location which provides suitable day to day living facilities. A locally based network co-ordinator (floating support worker) is commissioned to provide the required level of support to the individuals to make it a functioning network. This worker will have a specific brief to maximise their ability to live independently and will establish the “peer support” model where individuals will cross support each other within the Network e.g. one person may be good at cooking, another at the transport system. A local network co-ordinator will use their own community connections to support service users in building theirs.. Keywords: learning disabilities, mental health, physical disabilities
