Review of Hospital Discharge Pathways and Processes

Tender description

The overall objective of this piece of work is to inform and shape the City of London Corporation’s post hospital discharge processes and pathways to ensure that they are robust, fit for purpose and are meeting need effectively. This includes  Undertaking a review of existing City of London processes related to hospital discharge and some of the associated services such as reablement plus.  Identifying any gaps in services and processes and undertake options appraisal for meeting these gaps. This should include utilising any other services and resources within the local system  Developing any agreed processes and work with the Adult Social Care Team to embed these  Developing outline specification(s) for any services which need to be commissioned to start in April 2019.
