Tender description

The 0-19 Years Public Health Nursing Service. The 0 — 19: Public health nursing service will combine the functions of the health visiting and school nursing services to create a team that will work together to deliver the Healthy Child Programme (HCP) for children young people and families that has a family centred approach and has a focus on prevention and early intervention that contributes to the improvement of children and young people’s health and a reduction in health inequalities. The 0 —19: service will work across a number of stakeholders, settings and organisations to lead the delivery of the Healthy Child Programme (“the HCP”). It is expected that the supplier will utilise the combined team to reduce duplication across the workforce and improve efficiency for both the service and services users alike. The 0 — 19: service will support an integrated approach to meeting the needs of service users and the delivery of improved outcomes. The supplier will lead delivery of the HCP and work in partnership with maternity services, local authority early help services, voluntary, private and independent services, primary and secondary care, schools, and children's social care services. The 0 – 19: service will work closely alongside other professionals (including early years practitioners and early help staff, voluntary organisations, peer supporters, GPs and primary and secondary care providers as well as family centres and early years staff, social care and education) to ensure a holistic service focused on improving health outcomes, reducing inequalities at individual, family and community level.
