Tender description
Bradford Council and the three Bradford Clinical Commissioning Groups are seeking a provider who can develop and deliver an outcomes focused support service for Young Carers and their families and awareness raising and training on the identifying and understanding the needs of Young Carers. An outcomes focused support service for Young Carers and their families, including undertaking assessment, developing personalised support plans, and to provide a range of support and interventions to meet the differing levels of need and support required by a range of young carers and their families. Awareness raising and training on identification of and understanding the needs of Young Carers for a range of services including schools, school nurses, primary and secondary care providers adult services, children services and voluntary sector organisations.
