Tender description

Oldham Borough Council (“Authority”) and NHS Oldham Clinical Commissioning Group (“NHS Oldham CCG”) are working in collaboration under the banner of Oldham Cares and wish to consolidate arrangements for the provision of Oldham Care at Home, Extra Care and Specialist Care At Home Services by establishing a framework for the appointment of providers (and additional providers during the term of the framework) for the periodic placing of orders for these care services (“Flexible Purchasing System”) (“FPS”). Oldham Council is acting as the Flexible Purchasing System manager and lead commissioner for Oldham Cares on behalf of itself and NHS Oldham CCG. Care at Home services provide assessed eligible personal and social care to individuals over the age of 18 living in their own homes. Providers work closely with commissioners and social work teams, including crisis and emergency teams to ensure the timely delivery of care and support. The provision supports independence and reduces the need for more intensive services, including residential care and hospital admission, and supports discharge from hospital and intermediate care. We are looking to commission one provider for Extra Care Housing across all six schemes to ensure consistency and retain our focus on the development of the extra care service. We require an extra care provider who is experienced in working in partnership with housing providers, and understands the responsibility and issues relating to managing care on site based provision. The care provider will also be responsible for developing close relationships with the cluster based teams in which ECH is based, and other relevant health and social care professionals. Oldham’s vision is to be a place where children and young people will thrive. This will be demonstrated by children and young people with SEND achieving well in their education, finding employment, leading happy, healthy and fulfilled lives and having choice and control over their support. This category includes areas of specialism within care at home provision, which require additional skills to undertake more complex tasks, and areas where significant experience of the client group is essential. This is the cost of purely CHC funded client in their own home and not including residential of supported living The service user group will therefore include: Adult clients with complex healthcare needs who meet the criteria for Continuing Healthcare (CHC) funding which can encompass several sub categories: — End of Life care and support for those clients deemed to be end of life and rapidly deteriorating, — Mental Health and eligibility for s117 aftercare arrangements, — Clients with Learning Disability, — Acquired Brain Injury with Neuro-rehabilitation, — Spinal Injuries. The Approved Provider list will consist of all the providers who have successfully completed the SQ. For the avoidance of doubt, this includes all category winners and a ‘Back-Up List’ for relevant categories. The process for utilising the ‘Back-Up List’ will be explained in the mini-competition pertaining to each category. Only care providers on this list will be offered any business by the Commissioning organisations through this purchasing system. New providers will be able to enter the list, on a regular basis when the purchasing system is re-opened. Mini - competitions will be run from the approved provider list.
