Tender description

Dorset County Council (DCC) and NHS Dorset Clinical Commissioning Group (CCG) are developing a new Framework arrangement under Schedule 3 — Light Touch to provide high quality care and support interventions for people with learning disabilities in the community. The commissioning partners are committed to delivering high quality services and outcomes, whilst working within decreasing budgets. The aims are to deliver key outcomes in adult social care which are statutory responsibilities under the Care Act 2014: 1) deliver good health and care — affordable, flexible and accessible; 2) promote independence — helping people to help themselves; 3) enhance wellbeing for local communities. The Framework will replace the DPS for learning disabilities which has been in operation since 1.6.2016. Not all lots will be released at this time as they are provisional pending decisions by the new Dorset Council and are put here as a placeholder. These lots are marked as early provider information. Service providers are required to support individuals with daily living skills, community access, accessing health services and maintaining a good life quality. Supporting individuals to maximise their potential for independence and improve overall well being. Support young people and adults with a learning disability and/or autistic condition e.g. those who display self-injurious or aggressive behaviours, have specific neuro-developmental syndrome and where there may be an increased likelihood of presenting with behaviours that challenge. This lot, when released, will provide opportunity for providers of children’s support services to access the Dorset Care Framework for People with Learning Disabilities. Support will be available to children and young people. Live-in care provides support to individuals who wish to remain in their own home whilst in receipt of an agreed level of care and support. This service allows individuals to maintain their independence and their place in their chosen community. The support will be provided 7 days per week with the required breaks and days off built into support staff contracts, alongside cover arrangements for that absence. Day opportunities providers will deliver support to include community activities either individual and in groups of like minded people. It is envisaged that more people will access social and leisure activities as part of their support, moving away from building based activity for much of their support. Short breaks service is being developed to introduce greater choice for individuals with learning disability and their family carers. We are seeking innovative solutions to demand, and a transparent process that shows equal access to breaks for all. This lot is marked as early provider information. Employment service, when released, will provide a clear pathway into employment for people with learning and/or physical disability, and will be available to young people 16 years plus and adults. The service will provide a full service including learning about the world of work, vocational profiling, preparing CV’s and job applications and job coaching. Support to prospective employers will form part of this service. This lot is marked as early provider information. Community Hub and Spoke accommodation with support, when released, will provide independent living for people with learning disability with the additional opportunity to seek support from an On-Call member of staff. Innovative support solutions, when released, will provide a community-based brokerage and support planning support service to individuals wishing to purchase their own care and support using a direct payment, health budget or individual support fund. This service will provide accommodation, care and support to adults with a range of support needs including; learning disability, physical disability, autistic conditions, behavioural difficulties and a range of associated syndrome linked to their learning disability. Individuals may have multi-layered health conditions that might involve clinical oversight and/or regular nurse interventions. The learning disability partnership board is an important conduit for information sharing to all constituents with learning disability and their families. Equally, it provides a formal coming together of members that represent the views of stakeholders and those individuals interested in service developments. This Lot also requires management and operation of the learning disability forums and self-advocate quality checkers.
