Support to Live at Home (STLAH) - Provider of Last Resort

Tender description

The Council is looking to commission a contract for provision of support to live at home services across the Borough. This contract will be for packages which require commissioning outside of the current STLAH Framework Agreement. The Support to Live at Home service is commissioned to deliver personalised, outcome focused support to individual Service Users, at the right level and at the right time. The overarching aim for the Service is for every Service User to achieve their maximum possible level of independence at home and to improve well-being. The Service will focus on the principles of enablement, recovery and self-care, helping each individual to gain or regain the skills they need to live independently and to improve resilience to deal with potential health and social care needs in the future. The STLAH Service is delivered across the Borough, divided into the following Localities based on Area Council boundaries: • Locality 1 Penistone • Locality 2 North Barnsley • Locality 3 Central Barnsley • Locality 4 North East Barnsley • Locality 5 South Barnsley • Locality 6 Dearne. Providers awarded to this agreement would be offered packages Borough wide. Interested Providers are invited to attend a bidder session on 24th of October 2018.
