Tender description
The Service described in this specification will deliver high quality education and upskilling to Sheffield primary care clinicians based on evidence gained from the analysis of referrals from all Sheffield GP practices. The service will center on joint clinical expertise and integrated working between specialist GP reviewers and their individual named Consultant mentors. The members of Sheffield’s health economy are committed to the development of seamless patient pathways of care. The provider will have in-depth knowledge of the local pathways, guidelines and local clinical services in Sheffield; it will use the intelligence from the CASES service to enhance and develop the integration of existing primary and secondary care clinical services and working relationships between Sheffield GPs and Consultants based at Sheffield Teaching Hospitals NHS Foundation Trust (STHFT), as well as other providers in the NHS and the independent sector. Local context: in 2014 NHS Sheffield Clinical Commissioning Group (SCCG) set out its commissioning vision for healthcare services over the 5 year period ending in March 2019. The vision aimed to establish primary and community care as the setting of choice for more services, founded upon primary and secondary care clinicians working together to deliver seamless patient care. To support delivery of the vision the Elective Care portfolio developed the Clinical Assessment, Services, Education, Support and Self-Care (CASES) model. This model builds upon previous work, both local and national. The clinical assessment element of the model was subsequently piloted via a referrals peer review and education pilot which was developed through collaborative working between NHS Sheffield CCG (SCCG), Sheffield Teaching Hospitals NHS Foundation Trust (STHFT) and Primary Care Sheffield (PCS), a not-for-profit organisation representing Sheffield GP Practices. Learning from the pilot has resulted in the current procurement outlined in this service specification. The service outlined in this document provides the education and clinical assessment of referrals elements of the CASES model. Therefore, for the avoidance of doubt: whilst this document will contain references to CASES throughout and providers must be clear that the service being contracted for here forms only the education and peer review of referrals elements of the wider CASES concept.
