Medway Council and NHS Medway Clinical Commissioning Group are seeking to procure a high quality, effective service to improve the speed of patient discharge from Medway Maritime Hospital (Medway Foundation Trust). Commissioners want to pilot a Homecare Bridging Service that provides domiciliary care to patients in their own home whilst a long term care provider is sought. The pilot is intended to explore how improved outcomes can be achieved by better patient flow.
