Tender description
The CCG is looking for expressions of interest from investors and providers (joint or separately) for this Devon procurement to support individuals aged between 35 and 64. The service will be delivered to achieve specific outcome metrics including reduction in secondary care costs. The Big Lottery will contribute a proportion of the funding for the achievement of outcomes via a grant offer through the commissioning better outcomes fund. The CCG intends to invite investors and providers to bid either separately or together and to develop an intervention aimed at meeting specific outcomes linked to secondary care cost reduction and individual outcomes.
