Hertfordshire County Council (the “Council”) went out to procurement for the provision of an early support discharge service for stroke. This is an award notice and this opportunity is now closed. Hertfordshire County Council provides a range of services, including: — social care for adults and children, — schools and education, — public libraries, — public health, — highway and road maintenance, — environmental services, — trading standards and household waste sites as well as a fire and rescue service. The Council has award the contract to one care provider to deliver stroke specific enabling support at home. The expectation is that provider staff will co-locate and work closely with and under the direction of the Multi-Disciplinary Team (MDT) which includes OT’s, physios, community nurses and social care managers to run Wynchlands Crescent. The MDT has 2 bases, 1 in East and North Hertfordshire and the other in Herts Valleys.
