Tender description
This is a Prior Information Notice published by NHS Arden and GEM CSU on behalf of NHS Erewash Clinical Commissioning Group (ECCG). ECCG wishes to engage and collaborate with suitably qualified capable of delivering a Place Based Co-ordinated Specialist Respiratory Service to the patients registered with member GP Practices of NHS Erewash CCG for which they are the responsible commissioner. The aim of this Expression of Interest is to: 1) Allow the market visibility of the proposed opportunity; 2) Support the CCGs informed decision making on the commissioning of this service; 3) To help refine the commissioning and procurement approach and; 4) To gauge the level of interest from the market of capable providers with capacity within Erewash to deliver the services illustrated above. NHS Arden and Greater East Midlands Commissioning Support Unit (NHS Arden and GEM CSU) on behalf of NHS Erewash Clinical Commissioning Group are inviting Expressions of Interest from suitably qualified providers capable of delivering a Place Based Co-ordinated Specialist Respiratory Service to the patients registered with member GP Practices of NHS Erewash CCG for which they are the responsible commissioner. The aim of the Place Based Co-ordinated Specialist Respiratory Service is to proactively manage people at risk of developing, or who have a diagnosis of respiratory disease, via the provision of a place based preventative approach to care. The objectives are to: — Support the prevention of respiratory disease, in particular Chronic Obstructive Pulmonary Disease (COPD) and pneumonia, — Support the early diagnosis of respiratory disease via active case finding, in particular COPD, — Support the accurate diagnosis of respiratory disease, in particular COPD, adult asthma and pneumonia, — Support the on-going care of people with a confirmed diagnosis of respiratory disease via a Multi-Disciplinary Team (MDT) approach to holistic care planning, education, appropriate prescribing and co-ordination of care inclusive of pulmonary rehabilitation and home oxygen assessments, — Support the early identification, and on-going needs, of people with advanced stage respiratory disease. The service shall be delivered by a multi-disciplinary team (MDT) within practices, where possible, or community venues. This team shall provide respiratory related support for the following; physiotherapy, occupational therapy, psychological, specialist nursing, medical, dietetics, smoking cessation, pharmacological, self-management, disease education, lifestyle/wellbeing and health professional education in collaboration with GPs, practice nurses and patient advocates. The MDT shall be the single point of contact for people at risk of developing, and with a confirmed diagnosis of, respiratory disease and their carers to support them proactively, helping to navigate to the right services and be seen by the right person, at the right time along with supporting health care professionals requiring advice and guidance. The aim of this Expression of Interest is to: 1) Allow the market visibility of the proposed opportunity; 2) Support the CCGs informed decision making on the commissioning of this service; 3) To help refine the commissioning and procurement approach and; 4) To gauge the level of interest from the market of capable providers with capacity within Erewash to deliver the services illustrated above. Bidders who have expressed an interest in this market testing exercise will have the opportunity to submit a separate response to the Contract Notice as applicable. The deadline for Expressions of Interest responses is by 12:00pm (noon) on 17.10.2018. Please note this is a market testing exercise and does not commit the CCG to carry out a procurement process. Current services will remain in place during this period in line with existing contracts and service level agreements.
