Provision of commissioned Public Health Services in GP Practices and Pharmacies for residents of Wiltshire and Wiltshire Council staff. General Practices must have a CCG J number. The Practice should be in Wiltshire or on the border providing health services to registered Wiltshire residents (Patients have a Wiltshire post code or work for Wiltshire Council and have an identity badge). Pharmacies must be registered with GPhC and be already signed up to Pharmoutcomes and have suitable premises to provide private consultations in Wiltshire or on the border. Patients must be registered as living in Wiltshire or be a Wiltshire Council employee.
