PRJ-673 Barnet all-inclusive Anticoagulation Local Commissioned Service (LCS)

Tender description

This Anticoagulation Service (LCS) will assist in Primary Care and community based services at scale to the registered population of Barnet for the management of patients aged 18 years or over, for the initiation of anticoagulation and monitoring, where agreed clinically appropriate . It will be offered in three parts ? Part A –Undertaking a pulse check on patients 65 years or over, that have not previously been identified and recorded with AF. (Practice level only) ? Part B - DOAC monitoring and initiation (Practice level or community provider or those practices which do not provide the service) ? Part C – Warfarin and DOAC monitoring and initiation (To be offered at scale to full registered population by community provider ) This service specification is applicable to parts, B and C for all the community provider, unless specifically noted otherwise.
