Tender description

North East Hampshire and Farnham Clinical Commissioning Group and Surrey Heath Clinical Commissioning Group ("The Commissioners") will shortly seek expressions of interest from providers interested in delivering a range of primarily adult integrated community based healthcare services for the registered patients in the CCGs' area. This covers the localities of Surrey Heath, Ash Vale, Aldershot, Farnborough, Farnham, Fleet and Yateley. The Commissioners have a clear strategy for the development of integrated service models for health and social care within each of its localities, and the community provider/s must be an integral part of this. These services will work together to support individuals, their families and carers by providing a fully co-ordinated community based service. The providers must be able to work towards the continual improvement of services in a way that integrates: • Health and social care • Physical and mental health • Statutory and voluntary services • Provision and commissioning activities • Personal and professional responsibilities The Commissioners wish to commission only from providers that are willing and committed to this level of partnership working. Service Scope The services that are to be included are set out in the Memorandum of Information (MoI) that can be accesed via the In-tend portal project number F12026 Integrated Community Services . To note, these are to be procured in three lots: Lot 1: Shared services across both North East Hampshire and Farnham CCG and Surrey Heath CCG Lot 2: North East Hampshire & Farnham CCG specific services Lot 3: Surrey Heath CCG specific services Term Three contracts will be awarded with one for each Lot. The start dates of the contracts will be 01 April 2020. It is expected that the contract will be awarded for a total of five (5) years with the possibility to extend for up to a further two (2) years. Value It is anticipated that total contract values will be Lot 1: £7.5 million Lot 2: £9.5 million Lot 3: £4 million At this moment in time this notice is only to engage the market and test the CCG's Commissioning intentions.
