Tender description
The provision for disabled and/or autistic people (14+) which helps to them to stay independent in their own homes and connected to the support available in their community. This provision will help people to increase their independence and wellbeing and help prevent the need for longer-term health and social care support. This will include support to: — look after yourself and live more independently, — get paid work or take part in training leading to work, and — be part of your community.
