Tender description
The Midlothian Wellbeing Service will be delivered by the Provider in partnership with GP Practices and NHS Lothian. The wellbeing service will be embedded in GP practices with the aim of supporting people with or at risk of long term conditions or common mental health problems to live well. The wellbeing service will help to bridge primary care, health care, social care and the wide range of community resources that are available in localities.
