Provider Engagement Event - Care Navigation and Social Prescribing Services in Kent

Does your organisation currently provide care navigation and/or social prescribing services? Are you considering delivering these services in the future? Care navigation and social prescribing involve, for example, guiding people through the health and social care system, providing information and advice, signposting to services that support their wellbeing, supporting people to maximise their income and connecting people to community resources. Kent County Council has been working with the Kent CCGs to design a model of care navigation and social prescribing that aligns to the STP. Following the market engagement sessions held in June and August 2018, we would now like to share further details regarding the model and procurement process for the service with you.
