North Lincolnshire Council is seeking information on the delivery/provision of a targeted support service for adults who have had a stroke and/or have dementia. The ambition is for the targeted service to complement existing services for Adults and their carers as part of the North Lincolnshire Single Organisational Model. The Council would like to understand what the market can offer to complement existing services. Full information can be found at www.yortender.co.uk North Lincolnshire Council is seeking to issue a contract for approximately 2 to 3 years to provide targeted support to people across North Lincolnshire who have had a stroke and/or are living with dementia. The service will complement existing services and aims to: — enable older people to have choice and control over their lives, to continue to be part of their local community and to remain independent for longer, — support those caring for relatives and to give them knowledge on the best way to understand, manage and respond to the needs of those for whom they provide care.
