Tender description

Rapid Response Domiciliary Care Service within the Northern Health and Social Care Trust The Northern Health and Social Care Trust is seeking to procure a domiciliary care Provider to deliver a Rapid Response Domiciliary Care Service to support Service Users in their own home. The Rapid Response Service will provide short-term domiciliary care, which should initially be for a period of up to 4 weeks. The Provider will provide domiciliary carers at short notice no later than 6 hours after referral. The service will assist in the prevention of admissions to hospital and expedite discharges from hospital settings.
