Tender description

Brighton and Hove City Council and NHS Brighton and Hove CCG wish to commission a service provider to design and deliver an integrated Ageing Well Service (AWS) for older people in Brighton and Hove to start on 1.4.2019. The AWS will be open to anyone aged 50 years and over but will target those older people who are identified as being most at risk of a decline in their independence and wellbeing. The service will deliver innovative primary prevention and health promotion to support people to age well. Potential providers should note that a contract is to be awarded for a period of 4 years with the option to extend the period of the contract for further periods up to a maximum of 2 years in total at the sole discretion of the council. The service will: — reduce social isolation, — reduce loneliness, — promote good health and wellbeing, — prevent ill health, — enable older people to remain independent for as long as possible. Potential providers should note that a contract is to be awarded for a period of 4 years with the option to extend the period of the contract for further periods up to a maximum of 2 years in total at the sole discretion of the council. The council has a maximum budget available for delivery of the contract requirements of 2 392 000 GBP over 4 years of the initial period. The council is unable to consider tenders which exceed this sum.
