Shropshire Council established a DPS to commission Public Health Services.Tenderers applied for admission to one or all lots. Lot 1: GP NHS Health Check Lot 2: GP and Pharmacy Help2Quit Stop Smoking Service Lot 3: GP and Pharmacy Help2Slim Adult Weight Management Service Lot 4: Pharmacy Sexual Health Services: Issue of treatment for Chlamydia Lot 5: Pharmacy Sexual Health Services: Issue of EHC Lot 6: Pharmacy Sexual Health Services: Issue of Chlamydia/Gonorrhoea Smartkits Lot 7: Pharmacy Sexual Health Services: Registration and issue of Condoms as part of the Local CDS Lot 8: GP Contraception and Sexual Health: Opportunistic Chlamydia Screening of 15-24year olds. Lot 9: GP Contraception and Sexual Health: Fitting and Removal of IUCD, IUS or the purposes of contraception only, Nexplanon Sub Dermal Contraceptive Implant Lot 10: Pharmacy Community Needle and Syringe Programme Lot 11: Pharmacy Observed Consumption Lot 12: Shared Care (GP) A 5-yearly check for eligible 40 to 74 year olds to assess risk of heart disease, stroke, kidney disease and diabetes, including personalised advice and support to help reduce or manage any risk. Expert advice, behavioural support and stop smoking medicines to help people stop smoking. Personalised advice, support and encouragement to help people lose weight and maintain weight loss. This service is to provide treatment of Chlamydia to Service Users within Community Pharmacies in accordance with the Patient Group Direction Criteria. This service is to offer Emergency Hormonal Contraception to any woman over the age of 13 years in accordance with local terms and conditions and the Patient Group Direction Criteria and having a postcode within the administrative area of Shropshire Council. This service is to offer a Chlamydia/Gonorrhoea Self Sampling Smartkit to any service user at risk of a Sexually Transmitted Infection. To enable Young people aged 13-24 years old having a postcode within the administrative area of Shropshire Council wishing to register to, or already registered to the local CDS, are able to access condoms. To increase access to Chlamydia screening opportunities by providing additional locations where asymptomatic young people aged 15-24 years can access testing and treatment for chlamydia. To improve access to Long Acting Methods of Contraception, by providing a fitting and removal service. Community pharmacies within Shropshire providing managed and reliable needle exchange services to service users, in line with NICE Public Health Guidance 52 (PH52). Pharmacist are required to register service users using service and record exchanges taken place on nationally used database (PharmOutcomes). Where service users decline to register to provide emergency packs to reduce the spread of blood borne viruses. Community pharmacies to supervise the consumption of the dispensed prescribed medications in specified instalments. The role of the pharmacist is to ensure each dose is correctly administered to the patient for whom it is intended. As part of the safe management of the service user the pharmacist is required to liaise with the prescriber, named keyworker and others directly involved with the service user, in relation to any queries regarding treatment. Service is provided in line with national clinical standards, commonly referred to as the Orange Book (2017) and NICE Technical Appraisal 114 (TA114) Pharmacist also have a duty to monitoring the service user’s response to prescribed treatment, ensuring service user safety, which may involve withholding treatment-ensuring communication with prescriber. The provision also supports access to drug treatment delivery, retention and completion. Provision of opioid substitute prescribing and health checks through GP practice in partnership with the local drug and alcohol service, currently Shropshire Recovery Partnership. Eligible service users are jointly managed through the Shropshire Recovery Partnership and the GP to provide opioid substitute treatment (OST) both pharmacological and psychosocial in accordance with the national clinical standards, the Orange Book (2017) and NICE Clinical Guidance 51 (CG51) and Technical Appraisal 114 (TA114). Aim of the contract is to improve the physical, mental health and wellbeing of the patient and engage them in a process of recovery within a shared care setting
