Tender description

1.1. Caerphilly County Borough Council (CCBC) are seeking to appoint an organisation to deliver specialist safe, stable short term supported accommodation and Housing Related Support (HRS) for vulnerable families/parents through the Supporting People programme. The aim of the Service is to deliver Housing Related Support that reduces the: — Incidences of tenancy breakdown and/or individuals losing their homes, — Levels of homelessness and incidences of repeat homelessness, — Use of inappropriate temporary accommodation, — Crisis admissions to hospital and delayed hospital discharges, — Number of emergencies resulting in intensive services being required. There are three parts to this service provision. Housing Related Support, enhanced Housing Related Support within the dedicated Accommodation based services and thirdly move on/resettlement Housing Related Support for those moving on from Accommodation; any variances will be separately identified for the purpose of the specification. The service will be available to those who currently reside in or, who are out of county and want to return to their former residence of Caerphilly County Borough. The Service is for parents or expectant parents, mums and dads, singles or couples, who are 16 or over only, where a wide range of Housing Related Support needs have been identified.
