Tender description
This opportunity is for the delivery of reablement contracts in the following areas of Hampshire: Basingstoke and Deane, Fareham and Gosport and Test Valley and Eastleigh. Reablement services help people learn or relearn the skills necessary for daily living that may have been lost through deterioration in health or a change in circumstances. Sometimes, due to an accident, illness or simply growing older, people may find daily activities a struggle or feel unsafe in their home. Reablement is designed to help regain the ability and confidence to do some or all of the things people used to – whether it’s cooking for themselves, bathing without help or getting to the shops. Reablement will help you get to the point where people can look after themselves better and remain independent in your their own home for longer. It’s not about doing things for people. It’s about giving the skills and confidence to get back to doing everyday tasks and activities themselves- in a way that is realist. This is a Call Off mini-Competition from the Help to Live at Home Open Framework. This service is for the short term delivery of reablement care within the home, for a duration of no longer than 6 weeks (typically 4-6 weeks) as identified by the service user’s needs as part of the assessment process. The REACT service will be delivered using the principles and practice of a strengths based approach and in collaboration with The Council’s Community Response Team. Lot 1: Estimated at around 262 941,97 GBP per year (based on 263 packages per year (March ‘17-March ’18 average +10 % for growth) at 180 GBP per package and an average of 5 weeks per package + 36 Non starters at 60 GBP + Second Carer costs. 525 883,94 GBP total contract value if full extension taken. This is a Call Off mini-Competition from the Help to Live at Home Open Framework. This service is for the short term delivery of reablement care within the home, for a duration of no longer than 6 weeks (typically 4-6 weeks) as identified by the service user’s needs as part of the assessment process. The REACT service will be delivered using the principles and practice of a strengths based approach and in collaboration with The Council’s Community Response Team. Lot 2: Estimated at around 330 671,09 GBP per year (based on 296 packages per year (March ‘17-March ’18 average +10 % for growth) at 180 GBP per package and an average of 5 weeks per package + 68 Non starters at 60 GBP + Second Carer costs. 661 342,18 GBP total contract value if full extension taken. This is a Call Off mini-Competition from the Help to Live at Home Open Framework. This service is for the short term delivery of reablement care within the home, for a duration of no longer than 6 weeks (typically 4-6 weeks) as identified by the service user’s needs as part of the assessment process. The REACT service will be delivered using the principles and practice of a strengths based approach and in collaboration with The Council’s Community Response Team. Lot 3: Estimated at around 195 555,65 GBP per year (based on 190 packages per year (March ‘17-March ’18 average +10 % for growth) at 180 GBP per package and an average of 5 weeks per package + 40 Non starters at 60 GBP + Second Carer costs. 391 111,30 GBP total contract value if full extension taken.
