Community Navigation Service - Southampton City CCG

Southampton City CCG are seeking the market's feedback to assist in the Commissioning strategy for the intended Community Navigation services. We are looking to commission potentially a single contract for a city wide service covering the service requirements as follows: • Community Navigation for adults with mental health problems • Community Navigation for adults with organic mental illness (i.e. people with a diagnosis of or with suspected dementia, which includes younger adults with early onset dementia) • Generic Community Navigation for all adults. We are also looking for a provider to deliver a strategic leadership function to promote and develop a more joined up offer for Community Navigation working with the services above and other providers across the city, either as part of the above contract or as part of a separate contract for community development. A service specification can be found in the attachments area alongside a Request for Information (RFI) form for completion. This process is designed to help the CCG to form a view of the best way to commission the service and is not the start of the formal competition process.
