Tender description

Bridgend County Borough Council’s Social Services and Wellbeing Directorate wishes to award a specialist supported living service for up to four Individuals (living in shared accommodation) with a learning disability and complex challenging behaviour, including autistic spectrum conditions. Bridgend County Borough Council’s Social Services and Wellbeing Directorate wishes to award a specialist supported living service for up to 4 Individuals (living in shared accommodation) with a learning disability and complex challenging behaviour, including autistic spectrum conditions. The Provider will work with Individuals, their Care Co-ordinators and relevant Multi-Disciplinary Teams, including Health Professionals to deliver a customised package of Care and Support for each Individual and to ensure that they are supported to make their own decisions, take control of their lives, experience the same range of opportunities and life experiences as other people in the community and maintain independence using a model of Active Support.
