Tender description
The Brokerage Service is a bespoke intervention to facilitate prompt hospital discharge where an individual requires long term residential / nursing care. The Brokerage Service will support Service Users who receive Local Authority funding, those who are funded through NHS Continuing Health Care and self-funders, who are not eligible for Local Authority funding but may need help and support to find an appropriate placement.
