Tender description

Pharmaceutical supply and medicines optimisation service to the community teams, GP practices and in-patient units of Humber NHS Foundation Trust. The pharmaceutical supply services include the delivery of appropriately and legally presented pharmaceutical products for in-patient use as well as discharge, short-leave and outpatient services. — supply and dispensing of pharmaceutical products, — delivery of pharmaceutical products to the premises of the Trust, patients’ homes and other pre-agreed locations, — dispensing and home delivery of clozapine, — ward / team-based medicines optimisation services, — emergency and unscheduled services. Clinical pharmacist-led and pharmacy-technician-led medicines optimisation services in line with NICE and other relevant national and local guidance on every HFT in-patient units and at community-based teams.
