Tender description
The Countywide All Age Carers Service shall deliver improved outcomes through the pro-active identification of carers; assessment of needs; production of support plans; review of the support plan and provision of support for All Age Carers in Cumbria. The Service is part of a whole system approach to the delivery of services, and as such requires agencies to collaborate to provide early intervention, prevent and delay Carers from needing health and social care intervention, aiming to improve the health and wellbeing of Carers and their families.
