Tender description

The facility is to provide support for those individuals who require higher end Reablement and who have potential to progress or regain skills in order to regain their level of independence. They may “step down” from a hospital facility once they are medically optimised or “step up” from the community where the level of support required can no longer be provided at home but they have potential to improve and progress in their level of skills and function enabling them to return to their own home. The facility is time limited whilst improvement continues to the point where the individual is able to return home with appropriate care and support or community Reablement services.
