Tender description

Continuation of an integrated contractual solution across local primary care provision comprising of: a Care Planning LIS delivered in primary care; delivery of a virtual ward and care navigation service, and enhanced Practice Nursing provided though General Practice. The overall service delivers multi-disciplinary and integrated anticipatory care in the community to support patients at high risk of hospital admission to prevent admission and provide care following hospital discharge. The Care Planning ensures GP led risk stratification and development of anticipatory care plans, subsequently managed by Enhance Practice Nurses. Care Coordination links GPs and Virtual Wards. This is an existing service which is to be externally reviewed to ensure best value for money to enable the CCG to achieve its commissioning outcomes for local patients. The procurement process assessed as most appropriate is the direct award of an interim contract, on the basis that the incumbent provider is the only provider with the capability to ensure continuity of such integrated primary care provision of this service. The outcomes from the review will be integral to the Integrated Care Partnership developments in Harrow.
