Care navigation and social prescribing

Invitation to a provider engagement session to learn more about the future commissioning intentions for care navigation and social prescribing services in Kent Does your organisation currently provide care navigation and/or social prescribing services? Are you considering delivering these services in the future? Care navigation and social prescribing involve, for example, guiding people through the health and social care system, providing information and advise, signposting to services that support their wellbeing, supporting people to maximise their income and connecting people to community resources. Kent County Council has been working with the Kent CCGs to design a model of care navigation and social prescribing that aligns to the STP and would like to share this model with you for your comment and input. The session will take place in Maidstone at the following time: Monday 18th June - 14:00-16:00 If you are interested in attending this session, please email communitysupport@kent.gov.uk to request a place. Please ensure you put “Care Navigation Provider Engagement” as the email subject heading and provide the following information: • The name and contact details of the attendee(s) • Does your organisation currently deliver care navigation and/or social prescribing services? The deadline for expressing your interest is midday, on Thursday 14th June. Places will be confirmed via email by Friday 15th June at the latest. Venue details will also be confirmed at this stage.
