Tender description

Chronic pain affects up to 28 000 000 adults in the UK. Axial spine pain, arthritis and other musculoskeletal conditions account for 3 of the top 12 disabling conditions globally. Treatment of persistent pain requires a biopsychosocial approach, where the goal is to improve function and quality of life. Limited service provision is a common obstacle to the optimal management of these conditions. Frequently, patients are seen across a variety of outpatient specialist services, waiting times are long and services disjointed with inadequate communication between specialities in primary, community and secondary care. Changes to the treatments available within the NHS have necessitated that this service be commissioned in a progressive accessible manner in line with relevant guidance. Arden and GEM CSU on behalf of all four Lincolnshire CCG’s wish to commission services from providers including Community Services, Mental Health Services, Acute, Tertiary Care and private providers. The CCG’s are committed to improving the care provided to patients, reducing health inequalities and raising the quality and standards of care whilst achieving a financial balance. Aim. To develop and commission CPMS that will provide early identification and intervention for patients with chronic / persistent pain. The service will be commissioned to integrate and co-ordinate elements of the services currently available, whilst developing a range of biopsychosocial services to address the needs of patients. Objectives: 1) Provide a service through a single point of access for referrals for the management of chronic / persistent pain at the commencement of the contract; 2) Clinically triage all referrals to ensure patients are seen by the right clinician at the right time at the commencement of the contract; 3) Deliver and manage a comprehensive range of locally convenient and accessible biopsychosocial community services at the commencement of the contract; 4) Deliver a comprehensive range of self-care / self-management tools and resources, advice and guidance and care plans for patients and health care professionals at the commencement of the contract; 5) Improve patients experience by reducing unnecessary referrals, improving patient outcomes through a co-ordinated, integrated and efficient pain pathway; 6) Improve access to chronic pain services and patient outcomes. Expected Outcomes. 1) GP engagement will improve; 2) Patients will be referred to the correct pathway at the right time; 3) Improved patient satisfaction; 4) The service provided will be safe, effective and delivered in accordance with stated performance requirements; 5) There will be effective interfaces between CPMS, referrers and providers; 6) Achievement of appropriate local and national performance targets; 7) Use of NHS E-referrals; 8) 100 % of referrals are captured and processed through CPMS single point of access; 9) Develop and distribute service user questionnaires for patients, referrers, secondary care and other clinical interfaces; 10) The service will evidence patient clinical improvements at discharge; 11) The service will evolve to meet the demands of the patient by receiving feedback and benchmarking outcomes against national indicators; 12) The service will produce and disseminate clinical referral guidelines and care pathways across both primary and secondary care; 13) The service will provide support to referrers to improve the quality of referrals; 14) The service will deliver a high level of referrer satisfaction; 15) The service will deliver a high level of patient satisfaction; 16) The service will deliver a high level of commissioner satisfaction.
