Tender description
The Local Authority have commenced a remodelling program for their Social Care services with the aim of developing a “Help to Live at Home” model that incorporates outcome based intensive home support with access to equipment and assistive technology. The new model will incorporate several services that have been traditionally tendered independently, so we will develop a new working ethos within these existing services, whilst introducing new services that will help the Authority to get to the full Help to live at Home approach in future. The Services include: — Personal Care at Home. — Reablement. — A Hospital to Home service. — Overnight Service. — Extra Care. — Telecare Response. — Specialist Services. This service is to be tendered using the open procedure, it may also be in lots.
