Further, to the previous Market Engagement exercise completed in August 2017, NHS Croydon Clinical Commissioning Group (CCG) is inviting suitably qualified and experienced Providers of Anti-coagulation Services to an Outcomes Framework Engagement Event taking place in Croydon on the evening of 7.6.2018. The overall aims of the Integrated Community Anti-coagulation service will be to: Provide a cost effective, standardised, clinically effective and safe Integrated Community Anti-coagulation service; Improve quality and clinical outcomes through standardisation of care across an integrated pathway; Enable patients to have equitable access to a range of medications and treatments in a convenient community or domiciliary setting; Provide comprehensive and ongoing education to support medication adherence and patient self-management. Value excluding VAT: 1 500 000,00 GBP.
