Event details for 12th Feb 2018 “Shaping the future for People with a Learning Disability and/or Autism” Date: 12th Feb 2018 Time: 1pm to 3pm Venue, London Borough of Hounslow, Civic Centre, Lampton Road, TW3 4DN Parking: Please note that there is a pay and display car park on the grounds of the Civic, spaces are limited. Description The Joint Commissioning in conjunction with Adult Social Care have embarked on a project which embraces the wellbeing principles of the Care Act and Transforming Care agenda. We are calling upon providers who specialise in developing bespoke models of care and support that can meet the needs of people with learning disabilities that exhibit complex needs that are unable to be supported in our mainstream commissioned services under LIFE. We are driving a system wide change on how we Implement and source accommodation to deliver a viable & sustainable resource outside of the existing commissioned services within the London Borough of Hounslow and we are calling upon you to help us create this. We are looking for providers who have a proven track record of supporting people with a range of complex needs and can deliver a service to identified clients from 16 through to adulthood.
