The service will support the principles of integrated working with the acute trusts, community health services, social care, primary care, and voluntary sector services to deliver a co-ordinated approach to supporting people and their family carers. To provide: information, advice, guidance, practical and emotional support for a time limited period. Suffolk County Council are currently out to tender for the provision of this service.
