Tender description

Medway CCG and Medway Council intend to procure a comprehensive Care Navigation service from a provider by the end of summer 2018 with a view for the service to begin on October 1st 2018. Care Navigation fits into the 10 High Impact Changes outlined in the GP Forward View. Care Navigation is part of Social Prescribing and as a fundamental supports patient’s in their journey through the health and social care system. Care Navigation is vital for those individuals who need support in locating and accessing the appropriate services for themselves and those they care for. The aim is to create a service which is integrated into the healthy living centres, GP surgeries and the community around Medway.
