Specialist palliative care: Delivering high quality specialist palliative care to Camden patients in their last phase of life where death is foreseeable. The service provision will have a mixture of inpatient and outpatient service for the following provision of care; — Terminal care, symptom control; — Specialist skills in pain management and control of other symptoms; — Specialist rehabilitation care; — Where possible social, emotional and spiritual support; — Respite services can also be provided but are outside the scope of the funding under this Contract and should be agreed between the parties on a case by case basis. The provider is expected to deliver the service in Camden to meet the national agenda to move care closer to home. Also, the provider is expected to undertake the medication cost within the package of care for the patients using the service.
