Mansfield and Ashfield NHS Clinical Commissioning Group and Newark and Sherwood NHS Clinical Commissioning Group (the “Commissioners”) are looking for a group of providers which will lead, coordinate and, in large part, deliver support and services for local people at the end of their lives – End of Life Care Services. In 2016 the Commissioners established the Better Together Alliance, an alliance of the Commissioners, Nottinghamshire County Council and a number of health and care providers. End of Life Care Services are within the scope of the services to be delivered by the Better Together Alliance. 2 of the providers from the Better Together Alliance have been identified to develop and transform End of Life Care Services together with other non-Alliance providers. The new service is to be called End of Life Together and aims to deliver the following: See end of Section VI additional information for the remaining Text. This is not a call for competition. The Commissioners have identified 2 members of the Better Together Alliance to work with other providers in the system to deliver End of Life Together. The 2 Better Together Alliance providers are Nottinghamshire Healthcare NHS Foundation Trust and Sherwood Forest Hospitals NHS Foundation Trust. The Commissioners have also identified a number of other potential providers of End of Life Care Services to work with the Alliance partners in the delivery of integrated End of Life Care Services. These other providers include 2 hospice providers in Mid-Nottinghamshire who play a critical role in End of Life Care Services. They are Nottinghamshire Hospice (a registered charity) and Beaumond House Community Hospice (a registered charity). These providers will together take the prime responsibility for the delivery of End of Life Together: utilising a capitated budget for End of Life Care Services across mid-Nottinghamshire. To the extent that other providers are required to support End of Life Together they will be funded from the capitated budget allocated to End of Life Together. End of Life Together will be underpinned by new services contracts between Commissioners and the 2 registered charities and by a “service operations manual” between the identified providers who form End of Life Together. All partners will be equal in decision making, avoiding the hierarchy of lead contractor models. The Commissioners believe that they have identified those providers who are best placed to deliver End of Life Together but are seeking to determine any interest from other providers who could provide the End of Life Care services and who are willing to make the commitment to collaboration and an alliance way of working. Provider groupings who believe they have the necessary characteristics to form End of Life Together should express an interest by following the instructions set out in Section VI.
