Trafford Council are looking to put in place a Flexible Purchasing System Agreement (FPS) for Homecare Services, comprising of 3 Lots: Lot 1: Homecare: To deliver a high quality Homecare Service. Lot 2: Stabilise and Make Safe (SAMS): The Stabilise and Make Safe (SAMS) model aims to bring a new approach to short-term (and possibly intensive) home care support. Lot 3: Innovation: Trafford are currently looking for homecare providers who want to play an active role in redesigning the support we offer at home. We are currently trialling different ways of working through targeted prototypes in key areas in Trafford. Lot 1: Homecare Services: To deliver a high quality Homecare Service to people who fall within 1 or more of the following client groups: older people (adults over 65 +), adults over the age of 18 with Physical Disabilities, sensory impairments, learning disabilities, mental health conditions / dementia and long term conditions in Trafford. Lot 2: Stabilise and Make Safe (SAMS): The Stabilise and Make Safe (SAMS) model aims to bring a new approach to short-term (and possibly intensive) home care support which increases residents’ independent living skills and the ability to self-manage conditions. The key driver behind the service is to reduce the resident’s need, in line with our Care Act responsibilities. Lot 3: Innovation: Trafford are currently looking for homecare providers who want to play an active role in redesigning the support we offer at home. We are currently trialling different ways of working through targeted prototypes in key areas in Trafford.
