Tender description

Youth Support is a Families First funded service which will support vulnerable young people to manage their own wellbeing. The service will deliver support directly and coordinate wider networks of support. The service will assess needs, plan support and achieve outcomes jointly with young people and their families in a co-productive way. The service will promote resilience. This tender forms part of the recommissioning of the Denbighshire Families First programme from 2018-19 onwards. Youth Support is a Families First funded service which will support vulnerable young people to manage their own wellbeing. The service will deliver support directly and coordinate wider networks of support. The service will assess needs, plan support and achieve outcomes jointly with young people and their families in a co-productive way. The service will promote resilience. This is an early intervention service which aims to prevent young people’ needs from escalating and to prevent crises occurring. The service will support young people and their families to achieve outcomes in the following areas: 1) Young people feel more positive, confident and well; 2) Young people look after their health; 3) Young people have good relationships; 4) Young people control risky or damaging behaviour; 5) People have the skills to be good parents; 6) Young people cope better with their own challenges; 7) Young people live in good homes; 8) Young people deal better with money matters; 9) Young people and their families learn and study together; 10) Young people work well with schools and colleges.
