Tender description
The provision of planned palliative nursing care covering multiple patients, in their usual place of residence who are in the end of life stage of their illness. The palliative care service will make available 27 655 hours of care per annum across a 24 hour period, 7 days per week, locally co-ordinated, to deliver care to patients in their usual place of residence to support discharge from hospital, support admission avoidance and enable more people to die in their preferred place of care.
