Placements in residential and nursing home for adults aged 65 years + who require long term support or respite care. Reablement provision for adults, for a time limited period. Further provisions or categories may be added in future as required. The categories above consist Residential or Nursing Home Placement for an Adult (65+ years) who require longer term support or short periods of Respite Care. Emergency Care to prevent admissions to Hospital where this is not entirely necessary and assist with facilitating Hospital Discharge in a timely and efficient process. The Council are keen to engage with the market in September 2017 for Winter Planning purposes.
