Tender description

NHS Scarborough & Ryedale CCG has commissioned a partially integrated multi-specialty community provider model for the delivery of a range of community based health and care services for its registered and non-registered service users. The services include community services which have been provided by a third party and some in-house CCG functions. It does not include primary medical services. This is a 5 + 2 contract which was subject to an affordability cap. A range of first phase services will be delivered from the service commencement date with second phase services deliverable from year 2. The required service model comprises a number of previously distinct services and new services to be delivered through an integrated model and with strong links to other local health and social care services and service providers. The services include: — provision of a customer service /access centre /single point of access, — prevention, health promotion and wellbeing services, — planned care and intervention services, — planned and ongoing care, — nursing services, — specialist nursing services, — home oxygen service (assessment and review), — fast response, intermediate care and re-ablement services, — re-ablement and community therapy services, — primary care frailty service, — community elderly medicine service, — continuing health care, funded nursing care and fast track services. The service will work in a joined up fashion with local primary care service providers and the North Yorkshire County Council as the local social services provider - both relationships will be supported by integration agreements.
