Tender description
A long term service providing up to 212 hours of support and care per week, with the ability to facilitate a sleep-in as and when required (to a maximum of one per night) to people living within Rumbridge Court. Services will be required to support people to achieve the following: — Minimise the use of residential care, — Minimise unnecessary in-patient stays, — Reduce reliance on the long-term need for social care interventions, — Increase an individual’s independence through developing social capital/support networks and the use of universal services, — Reduce and mitigate mental health crises, — Reduce incidents of homelessness of individuals with mental health problems and for those with learning disabilities, — Support the personalisation agenda and the use of Direct Payments and Individual Service Funds, — Support individuals to gain and retain employment through signposting to other specialist organisations, volunteering and educational opportunities. This procurement was run as a Mini Competition by Hampshire County Council for the purposes of inviting proposals from suitable providers on the Hampshire Framework for Shared Living and Learning Disability Support. The service will be focussed on supporting people in their recovery from mental illness and people with a learning disability. Emphasis will be placed on group work, exploring the use of assistive technology, holistic support and developing peer support. It is paramount that this service provides support to people with mental health problems and/or learning disabilities in the community and that the service provider works in partnership with key stakeholders. The service consists of 11 self-contained studio flats in Totton, Hampshire. There are extensive communal areas within the building which are provided for the use of clients and support staff. The contract will be for an initial term of 3 years. This may be extended for up to a further 2 years on a 3 + 1+ 1 basis subject to an annual review and the mutual agreement of both parties.
