Supported Living with a transitional element of Educational Support within Goldlay Square, Chelmsford

Essex County Council is seeking to secure a provider to deliver a supported living scheme with an element of transitional educational support within a suite of purpose built apartments in Goldlay Square, Chelmsford. The primary purpose of this commissioned service is to procure outcomes based support which delivers independence, physical and mental wellbeing and employment as detailed in the Care and Support Plan for individuals who have been assessed by Essex County Council as being eligible under the provisions of Care and Support Guidance 2014. The individuals in receipt of this service will be young people with Special Educational Needs and Disabilities, whose support plans will focus on the process of transition to adulthood. Their plans will place particular emphasis on the development of independent living skills, and outcomes related to employment, with a focus on relevant education or training.
