Tender description

The provision of domiciliary care services to support people to live independently in their own homes with appropriate person centred services. Domiciliary Care supports individuals in maintaining potential in relation to physical, intellectual, emotional and social capacity to achieve individual outcomes and improve quality of life. It links to a number of measures including: — people with care and support needs have an enhanced quality of life, — people are helped to recover from episodes of ill health or following injury, — people have a positive experience of care, — people are treated and cared for in a safe environment and protected from avoidable harm. This contract relates to the provision of Domiciliary Care Services (Lot 1) and Short Term Domiciliary Care Service — Hospital Discharge (Lot 2) for the following: Adults aged 18 years over and including all older people (65+); carers, adults with a physical impairment; adults with a learning disability, with an autistic spectrum condition, mental health condition, sensory impairment, adults who require palliative care, dementia, and challenging behaviours.
