Aims and objectives of service There are two different patient cohorts who fall within the remit of this newly commissioned service but the aims of the service apply equally across all patients. The aim is to maintain and enhance the quality of healthcare for the registered list of patients, providing comprehensive and integrated primary and community healthcare service which has readily accessible proactive and reactive elements in line with evidence-based research and NICE guidance. Key characteristics of this service will be that it is patient-centred and produces efficiencies and savings for sustainability and reinvestment in the community.
