Tender description

The key objectives of the evaluation are to: • Evaluate the impact of implementing the new s136 pathway and HBPoS specification across all areas of London. Demonstrating this against the new model of care objectives and service user ‘I statements’; outlined in appendix 1 and 2. • Provide evidence to support further implementation across the system and maintain continued support from stakeholders, ensuring sustainability of the new model. • Identify any unintended consequences on the s136 pathway, or the wider healthcare system, and understand where approaches may need to be adapted. • Evaluate the programme processes, including critical success factors, challenges and lessons learnt. • Demonstrate the impact of large scale pan-London transformation programmes on London’s health and care system and share learning and evidence to inform other pan-London transformation programmes.
